RECOMMENDATION:
Staff recommends the City Council authorize the appropriation of $127,020.00 for
the purchase of six 12-lead cardiac monitor units in the amount of $127,020 and
training of fire department personnel in their use, and approve the related budget
amendment. (This entire sum will be reimbursed by the Los Angeles County
Emergency Medical Services Agency as per the Cardiac Care Program Equipment
Agreement.
A budget amendment requires a 4/5ths vote.
BACKGROUND/DISCUSSION:
On September 5, 2006, the Board of Supervisors approved the ST Elevation
Myocardial Infarction (STEMI) Receiving Center Program. The goal of this program
is to identify 9-1-1 cardiac patients experiencing a STEMI type of heart attack, and
transport them to an approved STEMI Receiving Center (SRC) hospital for definitive
diagnosis and treatment. A STEMI patient’s outcome can improve if definitive
treatment is administered quickly.
As a result of this program approval, each advanced life support (ALS) unit must be
equipped with 12-lead electrocardiogram (EKG) capability and staffed with
paramedics trained in its use.

The STEMI program has proven to be successful and the Los Angeles County
Emergency Medical Services Agency (EMSA) has required all 911 EMS Providers in
Los Angeles County to implement a 12-lead EKG/STEMI program. This unfunded
mandate created a significant financial impact to individual cities, such as the City of
Culver City, which lack the equipment and training to implement such a program.
The Los Angeles City and Los Angeles County Fire Departments received grant
funding from the Annenberg Foundation to purchase 12-lead EKG equipment and to
provide training to implement a 12-lead EKG/STEMI Program.
The Los Angeles Area Fire Chiefs Association (LAAFCA) recognized the vital
importance of the STEMI Program. Fire Chief Jeff Eastman, representing the
LAAFCA, began negotiations with the EMSA in 2005 requesting funding to support
the STEMI program. The negotiations were successful and on September 5, 2006
the Los Angeles County Board of Supervisors voted unanimously to use 4 million
dollars of Measure “B” funds to reimburse the remaining cities, including the City of
Culver City, for equipment and training costs.
Three vendors have been involved with the LAAFCA and have developed group
purchase plans for the Los Angeles Area Providers. After evaluation, the
Department determined the Philips MRX 12-lead cardiac monitor would best meet
our needs to serve the Culver City community. This unit will meet the parameters
and pricing required by the Cardiac Care Program Equipment Agreement.
County Counsel requires the City of Culver City to enter into an agreement, the
Cardiac Care Program Equipment Agreement, with Los Angeles County for
reimbursement of City funds spent on equipment and training to implement the
STEMI program.
Upon implementation of the STEMI Program, using the 12-lead EKG monitor,
STEMI patients can be quickly identified by the paramedics and transported to an
approved STEMI Receiving Hospital for the definitive life saving treatment.
STEMI Receiving Centers will be required to meet identified criteria and apply for
approval. It is anticipated that Brotman Hospital will apply for STEMI Receiving
Center status.
FISCAL ANALYSIS:
The Cardiac Care Program Equipment Agreement requires that individual cities
purchase their own equipment by December 31, 2006 in order to qualify for
reimbursement. The LA County EMSA approved the City of Culver City forCity of Culver City, California

reimbursement for the purchase of six 12-lead cardiac monitors up to $17,000.00
per unit and training costs of $25,020.00 for a total program cost of $127,020.00.
The expense of acquiring the program equipment and training is requested as a
budget amendment to the City’s General Fund until reimbursement is received by
Los Angeles County under the Cardiac Care Program Equipment Agreement.
Should the City Council approve this request, the reimbursement is expected to
occur during the first half of calendar year 2007 (within Fiscal Year 2006/2007).
A budget amendment requires a 4/5ths vote.
MOTION:
That the City Council:
1) Authorize staff to enter into the Cardiac Care Program Equipment Agreement
with Los Angeles County, for reimbursement of six 12-lead cardiac monitors and
equipment training (including reimbursement of the $127,020); and,
2.) Authorize the City Attorney to prepare the necessary documents; and
3.) Authorize the City Manager to execute the agreement on behalf of the City; and
4.) Approve a budget amendment in the amount of $127,020 for equipment
purchase and training to be reimbursed per the Cardiac Care Program
Equipment Agreement. This item requires a 4/5ths vote.
