RECOMMENDATION:
Staff recommends the City Council: (1) accept the work performed by the
contractor, Malcolm Drilling Company, Inc., (2) authorize filing a Notice of
Completion for the Reconstruction of the Slope Adjacent to Cranks and Tellefson
Roads Project, P-867, (3) authorize release of the retention funds in the amount of
$331,166.51 to Malcolm Drilling Company after the thirty-five day lien period
expires, (4) approve a budget amendment to transfer $6,781 into the P-867
account for outstanding invoice balance for the project geotechnical engineer,
Geo-Environmental, Inc.
A 4/5th vote is required to approve the budget amendment.
BACKGROUND/DISCUSSION:
Several previous City Council reports containing detailed background information
are attached. The following is a brief summary:
On February 12, 2005, after near-record rainfall, the slope on the properties at
10747 and 10751 Cranks Road failed. The following properties were subsequently
red-tagged by City staff: 10745, 10747 and 10751 Cranks Road and 5722 and
5724 Tellefson Road. Additionally, Tellefson Road from Bernardo Road to Cranks
Road was closed to through traffic.
After significant meetings between the City, the residents, the State Office of
Emergency Services (OES), and the Federal Emergency Management Agency
(FEMA), on May 22, 2006, the City Council approved entering into a professional

services agreement with Geo-Environmental, Inc. for the design of the slope repair
and construction inspection services.
A settlement agreement was approved by the City Council with the property
owners in January 2007 for the construction of a 3-tiered soldier pile retaining wall
system for the subject properties.
On August 30, 2007, the City Council awarded a construction contract to Malcolm
Drilling Company, Inc., for reconstruction of the subject slope. In October 2007,
reconstruction of the subject slope commenced. The residential structures at
10745 and 10747 Cranks Road were demolished to accommodate a 200-foot long
crane and other heavy construction equipment. Also the overhead utility lines
were relocated.
Heavy construction began in February 2008 with placement of 50 foot long steel I-
beams to structurally support the reconstruction of the slope. Subsequently, wood
boards were installed between the steel beams. In May and June 2008, the
construction of reinforced concrete caissons and a bridging structure occurred to
protect the City of Los Angeles’ 100 inch diameter North Central Outfall Sewer
(NCOS) main. This work was completed in July 2008.
In August and September 2008 the soil on the slope was graded and compacted
behind the new retaining wall system. Also safety cable rails were installed on top
of the walls. Finally the slopes behind the walls were sprayed with hydroseed for
erosion control.
FEMA Funds
Currently, staff anticipates reimbursement of $909,296 from FEMA/OES for the
design and construction of the project. Staff is currently preparing the application
to obtain the reimbursement funding. Also, it is the opinion of the project
geotechnical engineer that the slope failure encompassed a larger area than
what was originally assumed. Accordingly, staff is seeking additional
reimbursement from FEMA/OES for a greater portion of the total cost of the
project.
FISCAL ANALYSIS:
The original contract amount with Malcolm Drilling Company, Inc. was $3,207,700,
This amount was increased by $103,965 (3.24% of initial contract amount) due to
change orders. Therefore, the final contract amount is $3,311,665. The change
orders were for the relocation of power lines, asbestos abatement for the City of Culver City, California

demolition of the house at 10745 Cranks Road, revisions to the construction plans
to comply with the City of Los Angeles’ requirements for construction over their
NCOS line, hydroseeding, and additional excavation for placement of the retaining
wall subdrain system. The final project costs are shown below.

At the meeting of August 30, 2007, the City Council authorized the Public Works
Director/City Engineer to increase the construction contract with Malcolm Drilling
Company, Inc. and the professional services contract with Geo-Environmental, Inc.
by an aggregate amount not to exceed $300,000 due to unforeseen conditions.
The final invoice from Geo-Environmental, Inc. exceeds this contingency amount
by $6,781. Therefore, staff requests the City Council increase the contingency
amount by $6,781 to account for this increase via budget amendment. A four-fifths
affirmative vote is required to amend the budget.

RECOMMENDATION:
Staff recommends that the City Council approve a budget amendment of $44,904
and a professional services agreement with Geo-Environmental, Inc. for the final
design of the slope repair behind the properties located at 10745, 10747, and
10751 Cranks Road and 5722 and 5724 Tellefson Road.
A four-fifths affirmative vote is required to amend the budget.
BACKGROUND:
On February 12, 2005 a landslide occurred on the slope in the rear of the
properties located at 10747 and 10751 Cranks Road and 5724 Tellefson Road.
After consultation with a geotechnical firm, the Public Works Department and
Building and Safety Division deemed five properties (including 10745 Cranks Road
and 5722 Tellefson Road) to be unsafe for human habitation and all were 'red-
tagged.' Also due to the imminent threat of further sliding, Tellefson Road was
closed between Cranks Road and Bernardo Road.
Portions of this slope were subjected to landslides in the 1960's, 1978, late 1980's,
and in 1993. There have been several variations of repairs conducted throughout
the years that were funded by the Federal Emergency Management Agency
(FEMA) and the property owners. The most recent repairs to the slope occurred in

1993 when a soldier-beam pile system consisting of three walls, a series of
terraces, and concrete gunite were installed.
The failure occurred under the concrete gunite that was placed in the rear
properties at 10749 and 10751 Cranks Road and 5724 Tellefson Road. The slope
further east at the rear properties of 10745 Cranks Road and 5722 Tellefson Road
did not fail; however, it was the opinion of the City's consulting geotechnical
engineer that there was an imminent threat for a slide to occur there.
March 2005 to June 2005
There were several meetings held with FEMA representatives and the property
owners regarding repairing the slope. FEMA stated that the February 12, 2005
event was included under the federal emergency declaration of the January 2005
storms. Also, they recommended that an extension be filed to ensure that the
window of receiving possible federal reimbursement for the repair work did not
expire after the first 6 months following the date of the disaster declaration (August
4, 2005). Staff filed a twelve month extension to complete the repair work. At the
same time it was determined that a geotechnical and geologic investigation of the
slope should be conducted.
On June 10, 2005, the City Council approved a budget amendment of $35,000 and
a professional services agreement with Zeiser Kling Consultants, Inc. for the
geotechnical and geologic investigation of the slope behind the five properties
The work consisted of the following:
1. Research existing geotechnical reports, geologic maps and reports, aerial
photographs, and meetings with staff.
2. Borings to a maximum depth of 80 feet.
3. Laboratory testing to determine pertinent engineering properties.
4. Engineering and geologic analyses to develop a geologic map and cross
sections for input into slope stability analyses and develop design parameters
for a soldier pile retaining system similar to existing walls.
5. Final Report and preparation of an RFP.

After two months of negotiations between the City and two property owners to
allow the contractor access to their private properties, exploratory drilling was
performed in August 2005 and a draft report was submitted to the City by its
geotechnical consultant on November 27, 2005. The report revealed that the main
cause of the slope failure was loss of friction between soil strata as a result of
percolation of large amounts of rain water during record storms in December 2004
and January 2005. The report recommended that a three soldier pile wall system
be constructed, (similar to the system of walls constructed on the adjacent slope in
1993), and extended across the properties at 10745 Cranks Road and 5722
Tellefson Road.
Meanwhile, staff continued to meet with FEMA and OES representatives regarding
the subject matter. FEMA's review of the draft geotechnical report resulted in
questions regarding the reason for extending the repair across the slope where
there was no failure. Since the initial slope failure, the slope has not failed,
however, the Public Works Director/City Engineer and Building Official continue to
keep these two properties red tagged due to a possibility of failure and as
recommended by Zeiser-Kling. The response to FEMA was prepared by Zeiser-
Kling and in summary stated that the adjacent slope experienced headward
enlargement or escarpment due the adjacent slope failure, as evidenced by a
growing widening cracks on the property line walls between 10743, 10745 and
10747 Cranks Road. Also, Zeiser Kling conducted a slope stability calculation and
determined that the slope behind the properties at 10745 Cranks Road and 5722
Tellefson Road has a factor of safety of less than 1.5 and with an elevated
groundwater level, would reduce to less than 1.0, similar to the condition of the
adjacent slope prior to failure. In addition it was concluded that the slope is
experiencing ongoing creep.
December 2005 to May 2005
Staff requested Zeiser Kling to submit a proposal for the final design of the soldier
pile system and submit a construction cost estimate. Zeiser-Kling, along with their
civil engineering sub consultant, Fuscoe Engineering of Irvine, California,
submitted a $425,000 cost proposal for the final design of slope repair. Also, they
estimated that the slope repair will cost about $1 million.
Because the proposal for design from Zeiser-Kling was much more costly than
anticipated, staff decided to request multiple proposals and issued a Request for
Proposal (RFP) to fifteen geotechnical design firms for the final design of the slope
repair. Only two firms responded, David Lee Associates and Geo-Environmental,
Inc. The other design firms were contacted and they declined to submit a proposal

Upon review, David Lee Associate's proposal was determined to be non-
responsive because it did not include a final design of the repair and focused only
on the geotechnical analysis. Geo-Environmental's proposal was found to be
responsive and complete and they provided a total cost proposal of $118,000. The
proposal included material testing and inspection services during construction in
the amount of $30,080. Staff is recommending that this portion of the work be
awarded at a later date to coincide with the actual construction of the repair. With
this portion of the work removed from the contract, the final total cost proposal is
$88,550.
Under the current project worksheet approved by FEMA, the design is required to
be completed by August 3, 2006. Therefore, it is important to move ahead and
award a contract to complete the final design.
FISCAL ANALYSIS:
It is recommended that funding for this work be allocated from unreserved General
Funds to Other Contractual Services 42300867. A 415 th vote is required to
approve this budget allocation.
FEMA has approved $82,150 as eligible reimbursement costs for design of the
repair of the slope failure. FEMA will fund $61,613.00 or 75% of the eligible cost,
and State OES will cover $15,403.00, approximately 19%. In addition to the
reimbursements just discussed, FEMA and OES have already paid to the City
$3,780.00 as an administrative allowance for the design process.
The City has already expended $6,000 and $32,150 for the preliminary
investigations and analysis by Group Delta Consultants and Zeiser-Kling,
respectively. The remaining total of FEMA/OES eligible reimbursement funds
available is $42,646. An additional $45,904.00 is required to fund the Geo-
Environmental, Inc. contract. The City potentially would need to absorb this
expenditure if an alternative source of funding is not identified. Staff will be
working with FEMA and OES officials in an attempt to get the approved amount of
funding for design services increased through an amendment to the FEMA
approved Project Worksheet. Additionally, a future agreement with the affected
homeowners may include provisions for paying for some or all of the shortfalls in
funding for both desi gn and construction of the repairs.
Future Repair Costs:
The construction of the repairs has also been approved by FEMA and OES for
reimbursement in the amount of $885,413.00. FEMA/OES anticipated

reimbursements, based on the percentages discussed above, are approximately
$832,280.00. FEMA/OES have also already paid to the City $39,767.00 as an
administrative allowance for the construction process. The City potentially would
again need to absorb an additional expenditure if an alternative source of funding
is not identified.
Also it should be noted that the actual cost of construction has not yet been
identified and it is therefore possible that the construction costs will exceed the
amount currently allocated by FEMA. Additionally, FEMA's approved scope of
work for the project currently differs from the scope of work that the City
recommends. For both of these reasons, there exists the potential for a significant
shortfall in funding for the construction phase. However, FEMA has left the door
open for a future revision to the Project Worksheet for the repair work to both
increase funding and to expand the scope of work. This will be discussed with
FEMA following completion of the design as FEMA has indicated that they cannot
make that determination without a completed design. Also, as stated earlier, in
working out the agreement with the property owners, the possibility exists for the
property owners to pay a portion of or the entire construction cost shortfall.

PROJECT GOALS
This project will provide repairs to the slope damage caused by the recent storms during the winter of 2004-2005 to
the previously repaired slopes following damage in the 1980s and 1990s. Geo-Environmental, Inc., in coordination
with its subconsultants, Wheeler & Gray, Inc. and Cabrinha Hearn & Associates, will provide engineering and
support services for the preparation of construction contract documents for public bidding for the projects.
The project site consists of a 30 to 75 foot slope between the City streets and residential lots located at 10743
through 10751 Cranks Road, and 5722 and 5724 Tellefson Road. The majority of the proposed repair areas are
owned by the private property owners. Based on the recommendations of the City's previous Geotechnical
Engineers, the Construction project will involve the demolition of the remaining slope facing construction of a two-
tier Cantilever Soldier Pile retaining wall system, backfill of the Soldier Pile wall, reconstruction of slopes as geogrid
reinforced slopes, construction of terrace drains, repairs of the existing undermined and damaged terrace drains as
well as repair of any damage to the existing Soldier Pile wall panel. However, the validity of the choice of the
provision of the two-tier Cantilever Soldier Pile retaining wall system as the most cost effective and prudent method
of slope repair will be independently verified by Geo-Environmental, Inc.
CRITICAL ISSUES
Critical issues for these projects are maintaining the construction budget within the available funds. According to
the City's RFP, the project budget for the restoration and rehabilitation of the damaged slopes, inclusive of all costs
of design, engineering, construction, permitting, fees, supervision and management is estimated to be $1.2 million.
The project site lies both within the City limits as well as private properties requiring polite interaction with the
property owners.
PRELIMINARY SCHEDULE
A preliminary schedule is attached at the end of this section.
ACCELERATION OF DESIGN AND CONSTRUCTION
Acceleration of the project schedule is best handled during design by determining construction methods and
techniques, project phasing, and project combinations that will serve to eliminate duplication of tasks, down time for
contractors' crews and equipment, and conflicts between critical path tasks during construction.
SCOPE OF SERVICES
Geotechnical Engineering
Geotechnical engineering will be conducted by Geo-Environmental, Inc. This task will independently determine the
cause or causes of failures with the purpose of designing repairs and restoration to eliminate, to the extent feasible,
the possibility of a similar failure in the future. In this process, the recommendations made by the City's previous
Geotechnical Engineer will either be validated or modified.
Environmental Documents
it is expected that this project will be categorically exempt under California Environmental Quality Act (CEQA) and

Engineering Survey
Topographic Survey will be performed by our subconsultant Cabrinha Hearn and Associates. Our subconsultant
Wheeler and Gray will research the City's records and research the utilities in the area. As a part of the utility
research, we will also ascertain if the utility purveyors have any future work that will impact construction.
Right-of-way Engineering and Acquisition Services
It is expected that no permanent new rights-of-way or easements will be necessary for the completion of this
project. Our fees in the Cost Proposal portion of this proposal are based on this assumption. Permission for entry
and temporary easements for construction are expected to be needed. Our subconsultant Wheeler & Gray will
prepare mapping and legal descriptions and Geo-Environmental, Inc. will conduct communication with affected
parties.
Preliminary Design
Our subconsultant Wheeler & Gray will prepare preliminary documentation to the extent necessary to prepare
environmental documentation, construction cost estimates, and provide review material for the City on which to
comment.
Final Design — Plans, Specifications and Cost Estimates
Final design will consist of completion of all documentation necessary to conduct a public bidding of the project,
including drawings, specifications, engineers construction cost estimates, right-of-entry permissions, and
environmental documents.
Geo-Environmental, Inc. will meet with the City of Culver City staff as required to review submittals and keep the
City continually updated on the progress and the status of the plans and work.
Our subconsultant, Wheeler & Gray, will contact utility firms and transmit preliminary drawings early in the project to
coordinate their anticipated work in the near future and to coordinate any potential conflicts.
Construction and Engineering Management
Gee-Environmental, Inc. will provide construction management and engineering services with the purpose to
ensure adherence to the design intent and timely prosecution of the work.
Materials Testing and Inspection
Testing and inspection in the field will be conducted by Geo-Environmental, Inc: on an as-needed basis in
accordance with the estimate of cost included in this proposal.

PURPOSE
GEI proposes to perform a soils investigation on the damaged slopes located at different locations to investigate the
cause of slides and provide recommendations for their stabilizations. This investigation will include the collection of
bulk and undisturbed soil samples from the site by drilling bucket-auger borings, laboratory testing to determine
pertinent geotechnical parameters, engineering analyses (slope stability evaluations) and preparation of a report
presenting recommendations for slope stabilization. The soil investigation reports will also identify any clayey
cushion, subsurface soil and bedrock conditions, which may be detrimental to the rehabilitation of the slopes. Our
scope of work is described in more detail below.
SCOPE OF WORK
The scope of our investigation to evaluate slope stability will consist of five tasks: (1) Background Data Review, (2)
Field Investigation, (3) Laboratory Testing, (4) Data Analysis, and (5) Report Preparation. These tasks are briefly
described below.
Task I - Data Review - Available geotechnical data regarding the subject site and surrounding areas will be
reviewed to assess the expected soil conditions at the site. The data review will include review of published
geologic maps, stereoscopic aerial photographs, and other information contained in our in-house files, and
City/County files.
Task 2 - Field Investigation - The field investigation will consist of advancing two (2) 24-inch diameter limited
access bucket-auger borings to an estimated depth of 30 feet below the ground surface. Based on our initial site
visit the heights of the slopes are estimated to be approximately between 30 to 75 feet. GE' will prepare benches on
the slopes to accommodate limited access drill rig. The boreholes will be downhole logged for in-situ soil and
bedrock conditions by a Certified Engineering Geologist. During our downhole logging, we will note the orientation
of the bedding planes and potential failure zones.
During drilling, we will collect bulk and relatively undisturbed drive samples of the subsurface soils. The drive
samples will be collected at approximately 5-foot intervals with a modified California split-tube sampler or a
standard penetration sampler. Bulk samples will be collected from soil cuttings generated during drilling. The
samples from the borings will be tested in the laboratory as described in Task 3.
Task 3 - Laboratory Testing - Samples collected during the proposed field investigation will be examined in the
laboratory to confirm field classifications. Selected samples from the proposed borings will be tested to help
evaluate engineering properties. The numbers and types of tests will depend upon the type of soils encountered.
For a list of anticipated laboratory testing please refer to Table 1- Fee Estimate.

Task 4 - EnAjneering Analyses - Results from the field and laboratory data and GEN experience will be the basis
for the engineering analyses. GE! will provide engineering conclusions for the following:
? Type and extent of materials encountered,
• Sliding zones and the potential for larger sliding in the near future,
• Compaction characteristics of on-site soils,
• Presence of groundwater or seepage,
• Suitability of onsite materials for use for slope rehabilitation,
? Nature of bedrock and soil strength characteristics, and
• Recommendations for slope stabilization
Task 5 - Report Preparation - GEI's conclusions, recommendations and supporting field and laboratory test results
will be presented in an engineering report. A draft report will be submitted to you for review and comment The
final report will address and incorporate the review comments as appropriate.
The report will present drawings depicting construction details with sufficient information for the contractor to
implement the most cost effective alternative for the slope repairs.

MATERIAL TESTING AND INSPECTION SERVICES

PURPOSE
The City intends to restore and reconstruct portions of the slopes and roadways located at different locations in the
City (10743 through 10751 Cranks Road, and 5722 through 5724 Tellefson Road). The reconstruction program
includes the removal of debris, damaged sections and all disturbed soils. The restoration of the slopes and
roadways will include the construction of two-tier canilever soldier pile retaining wall, which include some other
pertinent small structures as curbs and gutters. GEI will provide geotechnical observations and soil testing services
during the reconstruction of the retaining wall and pertinent structures.
SCOPE OF WORK
Task '1 - Field Observations and Soils Testing
2.1.1 Evaluation of excavation: GEI's Engineering technician shall inspect the bottom of excavations for the
keys and benches to ensure that competent material has been exposed for the placement of the fill.
2.1.2 Evaluation of earth material to be used as fill: GEI's engineering technician will collect samples of the
proposed fill material and take them to the soils laboratory. Appropriate soil tests will be performed to
determine whether the fill material is appropriate for use in the reconstruction of the slopes and for the
new pavement sections.
2.1.3 Evaluation of compaction procedures: GEI's engineering technician shall evaluate the compaction
procedure for the backiiii of the slopes to ensure that the project specifications have been followed.
(Appropriate thickness of lift, proper compaction method, etc.).
2.1.4 Testing of compaction, moisture content, etc.: GEI's engineering technician shall conduct compaction
tests by Sand Cone Test Method (AS•M D1556) and Nuclear Method (ASTM D 2922-01) to determine
the relative compaction of the fill material placed for the restoration of the slopes and roadways.
During the progress of our work, our field technician will prepare daily field reports. A copy of these reports will be
submitted to the Inspector from the City of Culver City on a daily basis. The report will include a written summary of
the day's activities, a summary of all field-testing performed, and a listing of outstanding failing tests which have not
been reworked/retested. In addition, the location of all field density tests taken during the project will be plotted on
a set of plans that is maintained by the field technicians. The Contractor will be asked not to place any backfill
material that does not meet project specifications.
Testing time and the associated fees can be minimized by proficient and timely work by the contractor and by
coordination among project superintendent, the City Inspector and GEI engineers and technicians. When a larger
number of tests are scheduled at one time, greater efficiency can be achieved, resulting in lower overall fees.

field and laboratory testing, and a conclusion as to the project's compliance with the contract plans and

engineering properties of the on-site soils, and provide recommendations for the rehabilitation of the streets. The subject streets have experienced pavement distress that
has occurred over a period of time and ranged from minor to severe. Mr. Givens supervised the field drilling and soil sampling operations in the field and logged the soil

basis during the relocation of the water transmission mains located under the State Route 60/Interstate 215 Freeway in the City of Riverside. In the field Gas technicians
performed density tests on the compacted soil during the backfilling of the jacking pit and receiving pit at the end of the pipe casings to determine the relative compaction of
the banal soils at the locations. Mr. Givens performed the required laboratory tests for evaluation of the compacted material. in the field, Mr. Givens performed the needed

Several landslides occurred in the City of La Canada Flintridge due to the record-breaking rainfall Southern California received in 2005. The
primary objectives of evaluation by Geo-Environmental, Inc. (GE]) were to characterize the nature of the earth materials in the distressed slopes, identify the mode of failure,
and provide advice and options for restoration of the slope, guardrail, and pavement surface. Mr. Givens assisted the Certified Engineeting Geologist in the field exploration
and soil sampiing operations. The field work included the logging of exploratory tenches, hand-auger and mechanically powered hollow-stem auger borings. He aiso
performed many soil tests in the laboratory, which were needed for the performance of the stability evaluation.
